What is your question? The name field ties to the label for= field. They need to match. Example: label for="shirt_size" has to match name = "shirt_size".

The problem was the end of the section </section> Had to move the code <label> Shirt Size:</label> <input type="radio" id="small" value="small" name="shirt_size"> <input type="radio" id="medium" value="medium" name="shirt_size"> <input type="radio" id="large" value="large" name="shirt_size"> below the </section> tag.
