This is what mine looks like

      <label>Shirt Size</label>

      <input type="radio" id="small" value="small" name="shirt_size"><label for="small">Small</label>
      <input type="radio" id="medium" value="medium" name="shirt_size"><label for="medium">Medium</label>
      <input type="radio" id="large" value="large" name="shirt_size"><label for="large">Large</label>

      <button type="submit">Place Order</button>

I could be wrong, but I believe your missing the second pair of select tags.

Not sure what the name and id were so I guessed

      <label> Shirt Size:</label>
      <select name="shirt_size" id="size">
          <input type="radio" id="small" value="small" name="shirt_size">
          <input type="radio" id="medium" value="medium" name="shirt_size">
          <input type="radio" id="large" value="large" name="shirt_size">
          <input type="radio" id="small" value="small" name="shirt_size"> Small<label for="small">
          <input type="radio" id="medium" value="medium" name="shirt_size"> Medium<label for="medium">
          <input type="radio" id="large" value="large" name="shirt_size"> Large<label for="large">
      </select>

Bummer! You need to add a <label> element with its "for" attribute set to "medium" within the <form> element. this is what i keep getting.

Alright so your labels are not closed and the sizes need to be inside your label tags like this ..

<label for="small">Small</label>
