Cannot create AVD (Android Virtual Device)

When I try to create the AVD, the "OK" button does not let me click on it to finish. I checked the SDK Manager and the images and all items for the Android 4.0 (API 14) are installed. Also, when I click on the Android Device Monitor, I get an error message that Java was started but returned exit code 13.
