Captions on areas of custom post types

I'm working on the Custom Post Types and custom fields. I'd like to put a caption in front of some of the custom fields. Is there a way to do this without modifying the content-author.php file? What is the preferred way?

This post would be a form in HTML, so I'm trying to mimic the field captions.
