configuring a template button

I have a template that I recently worked on to use for my site but I cant seem to understand how to configure the "Submit Now" button to go to my email that i created through my host. I am confused as to what is the appropriate code to use to configure my needs/ the email i need the end users email is info@deebeepics.net

here is the code for the button , I pulled it from dreamweaver and the index.html file
