Full code for the HTML Forms course?

I followed along with the course and wrote the full code out, but I accidentally deleted the files and cannot recover them. I like to keep code for future reference.

Would anybody be willing to post the full code for the Forms course? (html.index file).

I don't want to have to type out all the code again. Thanks!
