Grids & Responsive Grids

How do you determine the sizes and the layout before placing items on a grid. I understand how to use the grid, but I am curious how much work goes into the math, and sizing images, layout to make it look professional.

Not an exact science. I just draw it out on paper and try things until something looks good.
