How can I make a survey?

I want to make survey or something similar to a survey that gives results. For example http://dogtime.com/quiz/dog-breed-selector

What I need is for people to be able to answer questions and then with their question results, they get specific results. Exactly like the dog example.

If you know how to do this or know a site that can make this please let me know!

Google Forms could also be a great idea. I have used a couple times and it was simple and easy to use.

Drive -> Create -> Form
