How to add an item to an array

I am struggling to add an item to an array (at least I think that is what the question is asking). I have this so far, but I am unsure how to do it. Please can anyone explain what I am doing wrong.

while ($row = $results->fetch(PDO::FETCH_ASSOC)) {
    $item[$row["genres"]][] = [genre_id=>genre];
}

You've got $row in the wrong place:

while($row = $results->fetch(PDO::FETCH_ASSOC)) {
    $item["genres"][] = $row["genre_id"] = $row["genre"];
}
