Hi Atik!

You can create a form by using the tags 

 <form></form>

inside of that you can put your input fields and labels.

I would start with How to build HTML Forms. Once you have mastered that you can then take the PHP Development Track.

These will show both how to code your form in HTML markup and how to process that form with PHP.
