How to send preset form with picture like in the Ribbit app?

Hi guys,

I am developing a ribbit app, but than for two kind of users; suppliers and customers. Customers could fill a preset form (name/product/deliverytime/costs/picture), and send it to their suppliers (friends in the Ribbit app). Than the suppliers could react to this form.

I think to make it ike signupviewcontroller, the fields of the preset form, save it to the back end, and retrieve it later on. But I dont know how to include the possibility to capture a photo, to attach to this form? Could you guys give some ideas how to set up these things?
