HTML Forms, Select Menu Skill Test

Okay I'm doing the HTML Forms section(label, select, option, optgroup) and have made it to the skill test and cannot figure out what the quiz is asking for. I have done it 50 times basically coping what Nick says and cannot get passed 3/4. Can somebody please send me the correct code cause I don't have a clue what it wants.
