i wish to add form to website to accept email? which video is it in? or does anyone know how?

i am in index.html ,want to add webpage form to accept email and phone number from USER?

When working with forms I think you might want to use a back-end language i.e. Python(flask, django), Ruby(rails), NodeJS or PHP.
