MASH "SUBMIT" Button only refreshing page.

I'm about to finish the MASH game course. The only thing I have changed were a couple of colors, in which all I did was change some of the numbers in Hex Code; and also some of the text.

When I click "Submit" with or without answers, it just reloads the page, all form entires have been removed. It's like I just refreshed the page. Any suggestions?
