… then what needs to be added to the end of the web address?

http://localhost/contact-thanks.php?name=Alena
Query strings start with a ? and are comprised of name=value pairs separated by &s. So one might look like this:
http://localhost/contact-thanks.php?firstname=Alena&lastname=xxx&address=yyy&city=zzz
