I copied and pasted your code below, take a look at the comment. 

var id = "23188xtr";
var lastName = "Smith";
id.toUpperCase(); //this needs to be removed, the rest of the code is fine.

var userName = id.toUpperCase() + "#" + lastName.toUpperCase();
