Review my MASH website

So I just completed the Treehouse Club - Mash course and I want to share it for feedback. Just keep in mind that I am very new to this but I am wont be offended and encourage feedback.

I made my a questionnaire about wrestling because my wife absolutely loves it and I knew it would make her happy. Brownie points and learning code at the same time is awesome. TIA. Cheers!
