Have you checked out Harry Roberts' inuitcss? There's a good example there of how he structures files and I quite like it. His stuff is based on SMACSS, OOCSS etc. See inuitcss.com and click the "coming soon" link to see the new GitHub repo for the inuitcss rewrite.
