Simple contact plug in

I added the plug in and followed the video instruction. However, on my website, when I click contact I get the page that says (contact) instead of the boxes to add name, email, comment etc. What step did I miss?

I think the short code needed was [contact] in the example so using parentheses would only register as text and not code.
