styling signature lines with html and css

I need to display two lines and below the lines in subscript or smaller font put the word Signature and Title below the lines. Something similar to this: (first two lines would display signature and title below them)

Form completed by: _______________ ____________ Phone: ______________ Date: ___________

What's the best approach for this?

To get 'signature' & 'title' below the first 2 lines, use float & border-top. Then to draw lines inline with 'phone' and 'date' use display:inline-block & border-bottom.

is this an input form? or are you just displaying it for printing or something?

It's something they will print off and sign.
