Swift Actions and Outlets keep disconnecting. Is there any way to reconnect them?

Hi,

I keep trying to complete the Playlist App on the Swift course. However, every time i log back in and start up Xcode my Outlets and Actions are suddenly disconnected. Can anyone explain why? And can you reconnect these? Any help would be much appreciated as it is extremely annoying.
