Uploading Images

Ok guys so I need to make a form that lets me get a users name phone comments etc and then have them upload an image from there computer. I also need to send all of this information to a specific email. Can someone help me or point me in the right place for this.

Google Apps/Docs and personal google accounts let you create forms like that... they give you the code to paste on your site, and it will send to your email (as well as store itself in a spreadsheet on your google drive.

Dang I've been stuck on this all day and its that easy. So I can have a person go there to contact me and it will send an email to me if I choose my email?

Yep -- check out Google forms.

Otherwise, if you're doing Wordpress, Fast Secure Contact forms, Gravity Forms, Contact Forms 7, etc. -- all do that too.
