How can I create a button that will link users to a separate page in order to fill out the form. Having it on my home page has distorted my layout in my design. I just need a page for that purpose.

The easiest way to get to a new page is to use an anchor tag rather than a button. The beauty of this is that you can style the anchor to look like a button if thats what you are after with some simple css:
<!-- html -->
<a href="newpage.html" class="button">Fill out form</a>
/* CSS something like: */
a.button { padding:5px 10px; border:1px solid #CCC; border-radius:5px; text-decoration:none; }
