What is the answer to the question related to the form control element?

I do not understand what a form control element is

It is asking which element in the example code is a form control element...

Form controls include: -checkboxes -radio buttons -text input (ie. textarea, input) -menus -etc...

so the question is asking which of these form controls do you see in the example code...
