You'll need to expose your db somehow as an API. Do you have that set up already? If so, the basic ideas are exactly the same as what we cover in the Build a Self-Destructing Message App. The backend is a black box that can be custom, Parse, or whatever. The app will connect and call methods pretty much the same way, though the details depend on how the API is set up, of course.
