If we are to use BEM for elements that are 3/4 levels deep. How do we write that out?

For example do we write...

<form class="form">
    <label class="form__label">
        <span class="form__label-span"></span>
    <label/>
    <input class="form__input" />
</form>

Or do we write....

<form class="form">
    <label class="form__label">
        <span class="form__label__span"></span>
    <label/>
    <input class="form__input" />
</form>

Neither actually, as you aren't utilizing and properly using the BEM methodology! As it is to keep it as simplistic as possible. What you do is only use the block name followed by the element name like so:

        <span class="form__span"></span>
    <label/>
    <input class="form__input" />
