Setting up Gravity Forms Conditional Logic?

I'm trying to create a gravity form with conditional logic. I created the form and it works fine in the preview. When I call for the form in my template nothing will appear. I can do a function call for a different form fine. Is there an argument I need to add to the function in order for this to work. Here is the function I'm using right now.
