Can anyone see what is wrong in the hidden field's name attribute?

By looking at it, it looks like you've added the ' & ' Symbols, maybe change them?

Because you use the & symbols when wanting to use a special character. Maybe the file is thinking that?

I am too facing the same issue!! What was it added to overcome the & symbol?

These two:

 <option value="">&#8212; Select &#8212;</option>

They both have the & symbol.

Try changing the & symbol to this:
