Things seem not to be aligning well. Should I be doing something to change the form width?e

I can't seem to add a screenshot, but the x-close box appears below and not aligned to the rest of the form. Also, the checkboxes on the bottom are squeezed next to each other rather than a straight list down.

Hey can you post the complete problem? I can help you, but this code don't show anything to solve, post a photo of problem something like that, or the complete code.

Actually, I had some time to pour through the code and found that an extra </div> had been included. I guess it also put a break in the code in the wrong place.
