Upload MP3 files using php and storing in database

Hi, I tried to upload mp3 files using PHP. On clicking upload, an error occurs, that is undefined index: file from the following lines:

$fileName = $_FILES["file"]["name"]; $fileTmpLoc = $_FILES["file"]["tmp_name"]; $fileType = $_FILES["file"]["type"]; $fileSize = $_FILES["file"]["size"]; $fileErrorMsg = $_FILES["file"]["error"];

Please help me, its urgent. I am creating my website
