I included index.php, style.css, screenshot.jpg from parent folder. And reivse its style.css file path from Ultra folder.

Front End Web Development Techdegree Student 16,744 PointsDelete the index.php from the child theme. You don't need it. The only two files you really need to create a child theme are style.css and the screenshot for now.
Is your parent folder named "Ultra" or "ultra"? If you have the template listed wrong (case sensitive), this may cause the error message you are seeing. So if the parent folder is actually "ultra" you will need to spell it that way in the CSS line for "template".
