Why I input field Name and let field Email and Details blank but it doesn't show text request to input Email and Details

I have followed all the steps and reviewed. I input field Name and left field Email and Details blank but it doesn't show text request to input Email and Details. Seems it only works if field Name is blank. Anyone got same problem and can you advise this case. Thank you.
