You are missing closing quote while declaring "favorite_stuff" in the second <div>. Also, you are missing "type" attribute definition for each <input> tag. Here's a solution:

Yay, sorry for late answer, but @ srikarvedantam was darn right. Just add the closing quotes in the second class declaration at the end of favorite_stuff and in addition add the right type e.g. type="text" to every input inside the brackets.
