How to show other input type boxes when a particular radio button is selected?

I am developing a form in which when a user selects a particular radio button then other below input type boxes are shown to him.

You will need to use JavaScript / JQuery for that.

Check what the radio value is equal to and show other form items accordingly

Initially the input type boxes will be displayed as hidden so that it doesnt shows up on the page. Using Javascrip/JQuery match the value of the radio button and then display the hidden input boxes.
