if you run the code below you will see that the icons don't show. Could you please tell me what is missing?

In bootstrap there's no glyphicon-ca. You can check their codes here http://getbootstrap.com/components/

Is it the camera one you want to use? Its glyphicon-camera. I have checked your code and it works.
