3n+4 = 4 Is where we start, at the fourth element in this case;
Maybe you'll understand this easier this way:
- 3*0+4 => 4 nth element is going to be styled
- 3*1+4 => 7 nth element is going to be styled
- 3*2+4 => 10 nth element is going to be styled
and so on...

You were close with the nth-child. The correct way would have been.
img:nth-child(4), img:nth-child(3) { clear:both; }
This of course will only apply the clear before the 3rd and 4th child.
