The great unknown?

Hello Programmers. I am getting more and more into programming daily. Each day I read more syntax and watch more videos etc. But one question has still been bugging me... How are games made? Like... do you guys/girls always use Xcode for the IDE and do most things in-code, or do you use Autodesk along with it for the assets? Or do you use an entirely different IDE like Unity which is more visual? Is Unity a cheater way to program? How do I import custom assets like sounds and visuals? These are important questions I can't find answered the way I want anywhere. If someone would be willing to, please share!

they can be made with Unity for 3d gaming, Xcode using Swift or Objective C for iOS gaming or maybe Python or flash for web gaming
