When web designer start a project with a client. should the designer make the website with photoshop first ? then code it ?, and should it be with its exact width and height ? or just a preview for the customer ?, what are the steps of starting a web design with photoshop ?... I'm very good with photoshop.

I would first start with pencil and paper and figure out the navigation with the client. Sometimes clients have similar page which can be combined into one. Figure out their target audience and what their call to action is as well as making sure their brand stand out. After this is done I would do some simple wireframes to figure out if all the content is going to fit and also to show the first stages of user experience. Then after that's completed, I would start on photoshop files base of the wireframes. That's a long process but this makes the process most precise and avoids redoing multiple psd's. Some people like to go straight to psd but I like focusing on the user experience of my sites.

2,435 PointsId say check out illustrator basics as well, the teacher has some useful information at the beginning to tell about designing for websites.
He wont talk about clients, but the tech as web designer.
