What would the 11th entry be if I had an entry for each of the 5 columns?

It is saying that there should be 11 entries, which doesn't make sense since there are 5 columns.

If you are adding records the 11th entry would be the 11th row or record. It means your add was unsuccessful to a table that contained 10 records. If you post your SQL people can help further.
